Members must get their proof of membership from their company’s authorized personnel. Most of the time this is through the Human resource department. Depending on the company’s preference, members may be issued a physical card or a virtual or digital card downloaded from a mobile application.
DENTAL CARDS are used specifically for dental benefits availment in all HPDAI accredited clinics.
By simply presenting this card, our affiliate dentists will be able to accommodate you for dental services without the wait.
With its special features dentists will be able to know the available dental benefits you are entitled to without the need to call for approval.
Take note that validity of your card is indicated on the front. You must present a valid dental card and a valid ID as proof of identification everytime your visit our accredited clinics.
HMO CARDS are issued by your medical healthcare provider which means this can be used for both your medical and dental needs.
Each HMO have different dental providers, please check your cards. HEALTH PARTNERS should be the dental provider on your card. If no dental provider written on the card please call your HMO.
