Avail Your Dental Benefits

Make sure to visit the clinic at least 15 minutes before your appointment.

In case you need to cancel your appointment, please notify the clinic at least 24 hours before your schedule so the clinic can give the slot to other patients.

Upon arrival, present your proof of membership and valid ID. For new patients, you may be asked to fill up a patient’s record and dental availment form. Do not sign the availment form until after services have been rendered.

Once seated on the dental chair, inform the dentist about your dental concern.

The dentist, upon oral examination, may recommend one or more dental procedures to address your dental concern. Some may not be part of your dental benefits and will be charged to the you. It is best to ask for the fees of each additional procedure before opening your mouth for the treatment.

Feel free to ask for other options or be open to seek second opinion for your dental concern.
